Penis enlargement surgery or penile augmentation surgery, phalloplasty has sadly had its victims. Though since it's beginning in the '90 around 40000 men have used surgery as a penis enlargement method, and 5000 American men do so each year, phalloplasty should not be the choice for any man.
Though it is a recognized penis enlargement procedure, the honest plastic surgeons refuse to perform it, or have stopped performing it. The American Urological Association and the American Society for Aesthetic Plastic Surgery consider that penile enlargement surgery is high risk, its safety and effectiveness not being established so far. There are several techniques that can be used to surgically enlarge the penis. The lengthening and thickening are two separate procedures that can be performed either at the same time or individually. Procedure: Surgical Lengthening of the Penis During this procedure, the suspensory ligaments that connect the penis to the pelvic bone and that support it are cut. The suspensory ligaments in the penis not only support the penis and connect it to the pubic bone, but they also secure the half of the length of the penis from within the body. The ligaments are severed so that the hidden part of the penis moves forward and out, and more of the penis is seen. That means that the penis does not actually grow bigger but the part that was meant to be inside the body is forced out. If the procedure was successful in one patient, it can have horrible mutilating effects on the next one. No surgeon can predict before performing the surgery how it will affect the sexual organ. In the successful cases, flaccid length grows with up to one inch, though the most common growths are of about 0.5 inches. In 5% of the cases men do not experience growths in length or girth. In some of the cases the size of the penis actually decreases following surgery. The scar tissue in those cases shrinks and toughens after surgery. Prevention of the scarring tissue is the continuous stretching of the penis following surgery. If the continual stretching of the penis is not done, the initial gains can be lost. The cutting of the suspensory ligaments can also influence the angle of the erection in reducing it. Once the suspensory ligaments are cut the erection will most likely start pointing down in the worst case or straight forward. One of the most damaging effects of penis enlargement surgery is the fact that, by cutting the suspensory ligaments, the penis is no longer held firmly in place and injury can occur. Procedure: Surgical girth augmentation The girth augmentations are usually more successful than lengthening surgery. The most common grow in girth is with about 30%. Men usually gain 1 inch in flaccid girth and half inch to one inch in erect girth. Fat Transfer is one of the methods used to increase girth. This procedure was used since the beginning of the '90s and it is the collecting of fat cells from the patients abdomen or inner thighs, purifying of the fat cells and the injection of the pure fat cells in the shaft, along its length. The fat cells are injected between the erectile tissue and the skin of the penis. Unlike other surgical girth enlargement methods, fat transfer heals faster because no incision is made on the surface of the penis. However, the long term effects are unpredictable. The girth increase is lost after a period of time because some of the fat cells are reabsorbed by the body. In other cases, the fat will not reabsorb evenly on the surface of the shaft or will form nodules. If this happens, the penis will look lumpy or uneven. Most of the men who see these changes undergo further surgery to correct the damage. Also, the firmness of the penis is lost during erection, since the fat cells are softer than the erectile tissue. Dermal Fat Graft Augmentation is also used to improve girth. This technique is the removing of tissue from between the thighs or from the buttocks. The tissue is grafted with all the subcutaneous fat and then inserted under the skin of the penis. This kind of surgery is more stable in time than fat transfer. The amount of transplanted tissue that survives is bigger than it is in the case of fat transfer. More of the girth obtained through surgery is permanent since the grafted tissue is not as easily absorbed by the body. The surgery done this way however requires the grafting of tissue from the patient. The two skin grafts are usually 6 inches long and 2 inches wide. The recovery of the area is usually painful and the scars might remain. In some cases parts of the skin grafts might not survive or the grafts might contract, resulting in curvatures of the penis and asymmetry. The procedure lasts several hours and it can bring forth the risks associated with being under general anesthetic for long periods of time. Alloderm is also used to enhance the girth of the penis. Alloderm are thin sheets of cadaver skin. The skin is freeze dried, treated in order to prevent rejection and to eliminate the risk for diseases such as AIDS. The Alloderm strips are put around the penis just like the dermal fat grafts. More sheets of Alloderm can be put on top of each other until the wanted girth is achieved. The procedure takes less time to be performed and is less painful since the skin is not taken from the patient. However, as far as we are concerned, penis enlargement surgery is not an option to be considered. Besides the fact that the failure rates are high, it is painful, dangerous and the increases are small or inexistent. In some cases the penis actually shrinks.
